Patient Information

Advance Care Planning (Power of Attorney for Health Care):
On admission to our facility, you will be asked about advance directives. These are legal documents that allow you to give directions for your future medical care. It is your right to accept or refuse medical care. Advance directives help protect this right. They help make your choices known if you become physically or mentally unable to do so yourself. You will receive information about advance directives upon request and, if you desire assistance, Social Services are available to help you complete the documents.

Visiting Information

Western Wisconsin Health welcomes family and friends to visit any time. A visit from family and friends is often the high point of a patient’s day. To ensure quiet healing time for patients, others who wish to visit are welcome at the discretion of the patient and their medical team. Patient rooms are available with sleeper sofas to accommodate close guests wishing to be present with patients overnight.

After-Hours Access

Should after-hour visits be needed after 8 p.m. or before 7 a.m., please enter at the Emergency department entrance.

Cell Phones:

To assure patient care is of the highest quality and that our monitoring equipment is uninterrupted, cell phones are not permitted in patient care areas. Cell phones may only be used in the main lobby area or outside of the building.

Televisions:

Televisions are provided to our patients free of charge. If your room does not have a television, please ask the staff to assist you.

Wireless Internet Service is available to patients and guests. Please see the reception desk for the security access code.

Case Management:

A Registered Nurse is available to assist you with your discharge needs. The nurse is able to help obtain any home medical equipment needed, set up home care services, assist with nursing home placement, and/or provide you with information on community services.

Social Services:

If you have any concerns or anxieties that may require further assistance while you are hospitalized or after discharge from the hospital, please make us aware of the situation. Social Services are available at your request. They can also assist you with your discharge needs.
